Scholarship application form for 2019

Deadline for consideration is 5:00 pm., Tuesday April 2, 2019

Potential candidates for KSA Scholarships are asked to review the Eligibility and Selection Criteria before undertaking the application process.

 There is no pre-printed application form.  Your submission is to be typed (i.e. word processor or computer) and include the original plus five (5) copies of each component. (Use the link at the bottom of this page for complete instructions on what needs to be included.)

The first page of your application should contain all of the following:

YOUR FAX NUMBER(S) and E-MAIL ADDRESS(ES)  (if applicable)
YOUR SECONDARY SCHOOL - and involvement thereat
The UNIVERSITY/COLLEGE(S) you're considering attending and status of your application(s) to each.

INFORMATION ON ANY JOBS - you've had or have

DECLARATION:  You must include the following declaration on the first page:

 "I affirm that, to the best of my knowledge, all the information supplied with this application is accurate, and that my intention is to further my education with registration at a recognized post-secondary institution in the immediate autumn term. I acknowledge that a late or incomplete or inaccurate application will be rejected."

       The statement must be signed and dated.

Check List: A complete application includes this (the above) cover page and complete responses to each of the sub-sections (A), (B), (C) and the mandatory attachments (D), (E) as described below. You must submit the original plus five (5) copies for a total of six (6) applications to be considered.

Requirements: Please provide responses to each of the following five (5) sub-sections in a computer-generated submission. Your response to each sub section (A), (B) and (C) should be within the required range of 150 to 250 words. Scoring of the application will be based in part, on satisfying the word length requirement for the following categories:  
(A) Future: 
• Your plans for post-secondary education, including faculty of study and degree/diploma accepted to or pending. 
• Your career ambitions following university/college. 
(B) Minor sports commitment: 
• Your past, present and proposed involvement in organized sport(s), whether school-based or with community organization(s). 
(C) Demonstration of need for financial assistance: 

  •  Initiatives that you have taken to finance the cost of your post secondary education. List all scholarships, incentives and financial assistance applied for, pending and/or received, including amounts. You should provide an estimate of potential student aid (non-repayable grant and loan amount) available to you through OSAP {}.  Do not attach OSAP work sheets. 
  • Indicate if, and how, your parent(s)/guardian(s) are financially supporting your education costs. 
  • Indicate if your parent(s)/guardian(s) are currently financially supporting the education costs of other siblings in the household. 
  • Make evident any extenuating circumstance related to your financial need, but do not disclose or attach financial records, income statement or taxation documents for yourself or family members.
  • Make evident your need for financial assistance by considering all sources of financial support from parent(s)/guardian(s), bursary/grant/loan assistance and your monies saved, versus your tuition & student fees, books and lodging costs to complete your program degree/diploma/certificate. 

Attachments: All attachments must be included with your application in order to be considered.  
(D) Two “Letters of Reference” – one from each of: 
• A teacher from your secondary school. 
• An official (coach, etc) from a minor sport in “B” above. 
(E) The current (i.e. most recent) official transcript of Student Academic Record. 


Once you have all your application components all completed, and have made 5 copies of each page and attachment, you are requested to submit all the signed originals and five complete copies by the application deadline. Do not include any other documents in your application.

NB: The deadline date for submission of applications for consideration for the 2019 scholarships is 5 PM, Tuesday, April 2, 2019 at the MTE Kitchener office as noted below. Late submissions cannot be accepted.

The information collected by KSA in this application will be used only for the scholarship award program. The information will be kept confidential and will not be distributed for another purpose. KSA does not retain the applications beyond the selection process and awards dinner. KSA does not return applications to the applicant. All such applications are properly destroyed.
Services provided by MTE Consultants to the KSA are donated as community service. MTE is not a member, agent or partner of the KSA.
