Potential candidates for KSA Scholarships are asked to review the Eligibility and Selection Criteria before undertaking the application process.

 There is no pre-printed application form.  Your submission is to be typed (i.e. word processor or computer) and include the original plus five (5) copies of each component. 

The first page of your application should contain all of the following:

YOUR NAME
YOUR COMPLETE KITCHENER ADDRESS (including postal code)
YOUR PHONE NUMBER(S)
YOUR FAX NUMBER(S) and E-MAIL ADDRESS(ES)  (if applicable)
YOUR SECONDARY SCHOOL
The UNIVERSITY/COLLEGE(S) you're considering attending

DECLARATION:  You must include the following declaration on the first page:

 " To the best of my knowledge, all information supplied with this application is accurate, and I confirm my intention is to further my education with registration at a recognized post-secondary institution in the immediate autumn term."                          

       The statement must be signed and dated.

Use additional pages as required to cover the three requirements which must be addressed as indicated:  Note the guidelines as to length of each required response.

1. FUTURE:  (A) Your plans for post-secondary education (100-200 word summary)  and  (B) your career ambitions (further 100-200 words)
2. MINOR SPORTS COMMITMENT: A 100 to 200 word summary of your involvement and degree of commitment to minor sport(s) affiliated with Community Services, City of Kitchener. 
3. DEMONSTRATION OF FINANCIAL NEED FOR FINANCIAL ASSISTANCE: (A) Summarize the initiatives you have undertaken to finance you education and other scholarships/burseries applied for and whether status of submission.(100-200 words)    and (B) Indicate if and how your parent(s)/guardian(s), as appropriate, are financially supporting your education costs (100 - 200 words).

There are 3 also attachments which must accompany each copy of your submission:

1. SECONDARY SCHOOL REFERENCE:  A letter of reference from a teacher at your secondary school
2. MINOR SPORTS REFERENCE: A letter of reference from an official (e.g. coach) from a minor sport you noted you are/were committed to.
3. TRANSCRIPT OF ACADEMIC RECORD: Submit your current official transcript.

Once you have all your application components all completed (and have made 5 copies of each page and attachment) you are requested to submit all the signed originals and five complete copies  by the application deadline.

 NB: For 2010, Friday April 16 was the Scholarship Application deadline. The deadline for the 2011 program has not been set yet.
